Öğe Abdominal sakroservikopeksi operasyonunun yaşam kalitesi ve cinsel sağlık skorları üzerine etkisi(2023) Aksin, Şerif; Balsak, Deniz; Yılmaz, Mehmet; Aboalhasan, YasminAmaç: Pelvik organ prolapsusu (POP) cerrahisinde çeşitli yöntemler kullanılmaktadır. Bu yöntemlerden biri olan abdominal sakroservikopeksi başarı oranları bilinmesine rağmen yaşam kalitesi ile ilgili veriler yetersizdir. Çalışmamızın amacı abdominal sakroservikopeksi operasyonunun yaşam kalitesi ve cinsel sağlık ile ilgili sonuçlarını araştırmaktır. Yöntemler: Çalışmamız, Diyarbakır Gazi Yaşargil Eğitim ve Araştırma hastanesinde 01.05.2008 ile 31.12.2012 tarihleri arasında POP tanısı ile abdominal sacrocervicopexy operasyonu yapılan 72 hastayı içeren retrospektif kohort çalışmasıdır. Operasyon sonrası bu hastalar ortalama 29.68 ± 6.55 ( 20-49 ) ay takip edildiler. Hastalar, demografik, klinik operasyonel özellikler, cerrahi başarı oranları, vajinal semptom skoru, seksüel semptom skoru (FSFI), (POP Q) quality life skoru, komplikasyonlar, de novo şikayetler açısından değerlendirildi. Bulgular: Çalışmamızda yaş ortalaması 37.77 ± 9.18 yıl, parite 5.07 ± 1.83, operasyon süreleri 55.25 ± 8.55 ( 38-72 dk ) dakika idi. Hastaların hastanede kalış süresi 39.51 ± 7.03 ( 30-72) saati buldu. Mesh erezyonu hiçbir hastada saptanmadı. En sık görülen komplikasyonun sırt ağrısı olduğu izlendi.( %18,1 ). Ortalama 30 aylık bir izlem sonrası hastalarımızın vajinal semptom skorlarının ( p?0.001 ) ve seksüel semptom skorlarının ( p?0.001 ) belirgin derecede anlamlı bir şekilde düzeldiği sonucuna varılmıştır. Yine bu hastaların Prolaps ilişkili yaşam kalitesi (P-QoL) skorlarında anlamlı düzelme olduğu belirlendi. ( p?0.001 ) . Sonuç: Abdominal sakroservikopeksi; yaşam kalitesi, cinsel sağlık ve pelvik organ prolapsusu başarısı konusunda uygulanabilir, etkili ve güvenli bir cerrahi prosedürdür.Öğe Effects of pro-inflammatory interleukin-6 and anti-inflammatory interleukin-10 cytokines in pregnant women diagnosed with coronavirus disease 2019(2022) Göklü, Mehmet Rifat; Tunç, Şeyhmus; Aksin, Şerif; Andan, CengizAim: This study aims to investigate the relationship between serum interleukin (IL)-6 and IL-10 levels in coronavirus disease (COVID)-19 positive pregnant women and the course of the disease. Methods: In our study, serum IL-6 and IL-10 levels were measured in 28 third-trimester COVID- 19-positive pregnant women and 30 third-trimester healthy pregnant women. COVID-19-positive cases were classified as carriers or patients. While 13 of the pregnant women in the study group were asymptomatic or were being followed out due to mild illness; a total of 15 pregnant women, 7 of whom were in the intensive care unit (ICU), were hospitalized and treated. IL-6 and IL-10 tests of COVID-19-positive pregnant women were studied at the time of first application. Results: Seven (25%) patients with COVID-19 required admission to an ICU. The IL-6 level was found to be significantly lower in COVID-19-negative pregnant women compared to pregnant women who were COVID-19 carriers and patients (p=0.01). The IL-10 level was found to be signifi- cantly higher in pregnant women who were negative for COVID-19 compared to pregnant women who were COVID-19 carriers (p=0.002) and patients (p=0.002). Conclusion: Close monitoring of IL-6 and IL-10 cytokine levels is recommended to minimize the risk of adverse outcomes in pregnant women presenting with a suspected or confirmed diagnosis of COVID-19. In this way, it may be possible to distinguish moderate–mild COVID-19 from severe COVID-19 in pregnant women.Öğe Results of Cosmetic Gynecology Surgery of Our Clinic(2023) Yılmaz, Mehmet; Aksin, ŞerifDespite the increasing demand for the female genital system surgery, the data are insufficient. Our study aims to investigate the operations for female genital cosmetic surgery. 80 patients who underwent female genital cosmetic surgery within the indication in our hospital between 01.05.2018-30.09.2022 at Siirt University Training and Research Hospital were included in our study. Five types of operations were performed on the patients: labiaplasty, vaginoplasty, perinoplasty, hudoplasty, and hymenoplasty. Postoperative six-month satisfaction levels were measured. 80 patients were included in the study. 22 labiaplasty, 16 hudoplasty, 54 vaginoplasty, 64 perineoplasty, 3 hymenoplasty operations were performed. The mean age in the study was 36.2 ± 11 (20-75) and the mean parity was 3.4 ± 1.9 (0-8). The operation time was 49.8 ±9.1 minutes (35-80). No complications were observed in any of the patients. After the operation, these patients were followed up for an average of 6 months. Average patient satisfaction was 86% in labiaplasty, 100% in hudoplasty, 90% in vaginoplasty, 87 % in perinoplasty, and 66% in hymenoplasty. The results obtained in our study are that cosmetic gynecology can be performed within medical indications. It was observed that complication rates were low and patient satisfaction was good.Öğe Thyroid Function Tests and Thyroid Antibody Tests in Ectopic Pregnancies: A Prospective Cohort Study(Yuzuncu Yil Universitesi Tip Fakultesi, 2024) Aksin, Şerif; Yilmaz, Mehmet; Balsak, Deniz; Aboalhasan, Yasmin; Yoldaş, Adem; Batmaz, İbrahimThyroid hormone levels have been observed to induce structural changes in different regions of the fallopian tube. This study aimed to investigate the impact of thyroid hormones on ectopic pregnancy. In pursuit of this objective, 31 patients with ectopic pregnancy were monitored at Siirt University Training and Research Hospital between July 2022 and December 2022. Additionally, a total of 31 patients with normal first-trimester pregnancies, matched for age, parity, and gestational week, were included in th e control group. Various parameters, including patient age, pregnancy history, medical history, treatment methods, clinical and demographic data, TSH, fT3, fT4, TT3, TT4, Thyroglobulin, Anti-thyroglobulin Antibody, TSH receptor antibody (TrAB), Anti-Peroxidase Antibody, Anti-Tyroglobulin Antibody, hemogram, biochemistry, CRP, and sedimentation values, were compared.Clinical Trials no: NCT05446012 Statistically significant differences were noted in Free T3 (p = 0.011) and thyroglobulin (p = 0.018) values bet ween the ectopic pregnancy and control groups. Subsequent ROC analysis was conducted for the significant parameters, determining the AUC (Lower and Upper Bound), sensitivity, specificity, and cut-off values for each parameter. Multivariate logistic regression analysis followed, wherein the "backward stepwise model" was applied to the Free T3 parameter. The apparent parameter indicating an increased risk in the disease was Free T3, with a 4.2-fold increase and a 95% CI of [1.3-13.5]. Lower Free T3 levels were associated with an elevated risk of ectopic pregnancy. The assessment of Free T3 levels during pre-pregnancy counseling may aid in identifying pregnant women at risk of ectopic pregnancy. © 2024, Yuzuncu Yil Universitesi Tip Fakultesi.
